Det er hodepine – og så er det den typen hodepine som gjør at du får lyst til å krype under dynen og ikke gjøre noe. Hvis du vet hva vi snakker om, har du sannsynligvis hatt migrene.
Selv om mer enn 1 milliard mennesker lever med migrene over hele verden, er det ingen kur for tilstanden. Heldigvis er det mange måter du kan håndtere – og potensielt unngå – migrene og hodepine.
Vi snakket med Dr. Robert Fryer, klinisk professor ved Institutt for nevrologi ved NYU Grossman School of Medicine, for å finne ut de beste måtene å håndtere hodepinen på hjemme – og når det kan være på tide å se en lege.
La oss snakke om forskjellene mellom hodepine og migrene.
Vi kategoriserer hodepine i to hovedtyper: primære hodepinelidelser og sekundære hodepinelidelser.
En sekundær hodepinelidelse er når hodepinen er forårsaket av en annen prosess som foregår i hodet. For eksempel, hvis du har hodepine fra en bihulebetennelse, og du behandler bihulebetennelsen, vil hodepinen forsvinne.
Den andre kategorien er primære hodepinelidelser. Og det er der selve hodepinen er sykdommen. I dette tilfellet bruker du medisiner som direkte behandler hodepinen.
De to vanligste typene primærhodepine er spenningshodepine og migrene. På nevrologens kontor ser vi egentlig ikke mye i veien for spenningshodepine fordi folk bare tar Tylenol og de er vanligvis fine. Det er egentlig migrene som er den dårlige hodepinen som krever opparbeiding og behandling.
Er det visse grupper som er mer utsatt for migrene?
Migrene er en veldig vanlig lidelse. Alt fra 15 % til 20 % av befolkningen lider av migrene. Og rundt tre fjerdedeler av dem er kvinner.
Før puberteten er det faktisk litt annerledes. Det forekommer nesten likt mellom gutter og jenter. Hos yngre barn kan det være litt mer vanlig hos gutter. Men kvinnelige hormoner som starter i puberteten driver definitivt og kan utløse migrene. Så det er derfor det blir mye mer vanlig hos kvinner enn hos menn når folk blir eldre.
Hvordan kan en person identifisere forskjellen mellom migrene og hodepine?
En migrene er per definisjon en moderat til alvorlig hodepine. Så, det er en slags hodepine hvor det er vanskelig å gjøre ting, eller hvis det er veldig ille, kan du ikke gjøre noe. Du må bare legge deg i et mørkt rom med et teppe over hodet.
Men generelt sett er det en hodepine hvor det er veldig vanskelig å få ting gjort og det er vanskelig å konsentrere seg, vanskelig å jobbe. Hvis du har det noen ganger, bør du definitivt få det sjekket ut fordi det er noen veldig enkle ting som kan gjøres for å gjøre dem mindre plagsomme.
Hva er noen av de enkle tingene? Hvordan kan du takle migreneutløsere?
Det er fire hovedtriggere for migrene: sult, dehydrering, stress og mangel på søvn. Så jeg forteller folk at du må spise regelmessige måltider. Du må drikke vann hele dagen. Du må virkelig gjøre ditt beste for å holde en god søvnplan, og så må du finne ut måter å håndtere stress på.
Det neste trinnet er at noen personer med migrene faktisk vil ha en bestemt matvare som utløser migrene. Det er ikke veldig vanlig, men for noen mennesker kan det for eksempel være rødvin. Du vil ha folk som drikker et glass rødvin og får migrene. Hvis du kan identifisere en matutløser, vil hodepinen faktisk bli mye bedre ved bare å eliminere den maten.
Den beste måten å identifisere en matutløser på er å gjøre det som kalles en hodepinedagbok. Hver gang du får hodepine, går du tilbake og skriver ned maten du spiste omtrent fem timer før hodepinen begynte. Gjør det for noen hodepine, og så vil du ha en god følelse av om det er en matutløser.
Folk tenker kanskje å ta et par Tylenol, som du nevnte, men hva er noen andre alternativer hvis du føler hodepine eller migrene kommer?
Som en som har ignorert migrene i 30 år eller så, kan jeg snakke ganske bra om dette – jeg ville bare noen ganger tatt medisin for det og noen ganger bare ignorert det og presset igjennom. Generelt er det best å ta medisin.
Tylenol, eller paracetamol, er sannsynligvis det sikreste, men også kanskje mindre effektivt enn noen andre alternativer for hodepine. Men hvis det fungerer for deg, hold deg til det.
Ibuprofen, også kjent som Motrin eller Advil, er en anti-inflammatorisk. Så det er vanligvis nummer to på listen min å prøve. Et annet alternativ er en kombinasjonsmedisin som Excedrin Migrene. Og det er en kombinasjon av aspirin, som er en anti-inflammatorisk, og koffein. Og koffein kan være en ganske sterk, mislykket medisin mot migrene.
Du kan også ta en annen anti-inflammatorisk som naproxen. Og det kan være ganske effektivt for noen mennesker.
Og så er det reseptbelagte medisiner som kan brukes. Men jeg foreskriver dem bare hvis du har gått gjennom de andre medisinene og de ser ikke ut til å virke. Jeg synes det er lettere å ha noe hjemme som du kan få over disk.
Bortsett fra medisiner, hvilke andre behandlinger er tilgjengelige?
Botox er en nyere behandling. Det er en veldig effektiv behandling for det vi kaller kronisk migrene. De fleste har episodisk migrene. Det er da du har en fryktelig hodepine som varer i flere timer. Og så kan du gå en uke eller lenger før du får en til.
Kronisk migrene er overraskende vanlig – så mange som 1% av befolkningen har kronisk migrene. Og det er definert som minst åtte migrene i løpet av en måned.
Dette er pasienter hvor du tenker på Botox-injeksjoner. Og det er en serie på 31 injeksjoner rundt ansiktet, bak i nakken eller skuldrene, og de gjøres hver tredje måned. Men de er overraskende effektive for behandling av kronisk migrene.
